What data is collected

Only what you fill in yourself in the appointment request form:

• surname and first name
• phone number
• email address
• area
• reason for visit
• description of your request

Health data

The description of your request may include information about your health. Such data belongs to a special category and is protected more strictly.

It is collected only with your explicit consent, which you give by ticking the relevant box before sending the form. Please keep to the information needed for your request to be assessed.

Purpose and legal basis

The data is used solely to answer your request and arrange an appointment. The legal basis is your consent, which you may withdraw at any time.

Who has access

The data is never sold or passed on for advertising. Access is limited to:

• the data controller
• the form delivery service Web3Forms, which forwards the message to the email address
• the email provider hosting that address
• the hosting provider Netlify, which does not store the content of the form

Transfers outside the European Union

The form delivery service and the hosting provider are based outside the European Union. Your message passes through their servers only in order to be delivered to the controller's email address.

Transfers are made on the basis of the European Commission's standard contractual clauses.

Retention period

Requests that do not lead to an appointment are deleted after twelve months.

For people who start treatment, the details become part of the case file and are kept for as long as the legislation on health professionals' records requires.

Cookies and tracking

The website uses no cookies, does not track your behaviour and has no analytics or advertising tools. Fonts, map and video are served by the website itself.

Security

The connection to the website is encrypted. Form details are sent encrypted and are not stored on the website.

Your rights

You have the right:

• to request a copy of your data
• to request its correction or deletion
• to request restriction of processing or to object to it
• to receive your data in a readable format
• to withdraw your consent at any time
